Beginning January 1, 2018, there is a new Medical Power of Attorney form. If you click on this link, you can download the new form. If you are one of our clients, after you have signed the new form, please email me a copy of it or drop a copy by our office. We will update the information on your emergency document card and keep a copy for our records. You will use the old one for the remainder of December. Starting January 1, you will begin using the new one.
